  • If the answer is <YES>, then you will receive a notification that a special questionnaire will be sent to you to enclose your certificate. –> instruction will follow on the next page.
  • If the answer is <NO>, then a new field is opened where you must enclose a filled and signed questionnaire.

Click on the button <Submit Entire Response> and then on <OK>:
